McKenzie Medical Center is excited to have teamed up with Medicare to offer you the latest program in chronic care management. Chronic care conditions are a major cause of disability, illness and death in the United States.

Chronic conditions are diseases that are long-lasting or recurrent. Some of these conditions are generally not disabling. Others, however, can create significant limitations in a person’s ability to perform basic activities of daily living. This chronic care management is now available at McKenzie Medical Center. Our goal is to keep you on the pathway to better health while helping you manage your chronic conditions. This program offers a number of benefits for enrollees including:


1. 24/7 Access to a Care Team Member - This means if you are having an urgent health issue after hours, you can call and talk to one of our medical staff to address your problem. Our urgent need number is not for medication refills but is with a local provider who can help answer questions when you have urgent health concerns. Monday-Friday 7am-7pm and Saturdays 8am-5pm call the main clinic at 731-352-7907.

2. Same Day Appointments - These are with your provider if he/she is in the office or another provider familiar with your health issues when your provider is out.

3. Individualized Care Plans - These plans are tailored to your specific needs, with goals to help with your chronic conditions. You can review these care plans on our patient portal and at your office visits. Your provider will also review these with you to see if progress is being made on your goals.

4. Reminders for Preventative Care Procedures - When ever you have a preventative care procedure due such as a physical, immunizations and/or diagnostic tests such as a colonoscopy or bone density testing, MMC will send you a reminder so you can stay up-to-date.

5. Quarterly Newsletters - These will be filled with helpful information on how to live a healthier life and get the most out of your medical services.

6. Notifications of Additional Beneficial Services - There are a lot of service MMC offers and we are constantly adding and improving to our list. We will keep you informed about these services, which include Diabetic classes, free screenings and other wellness programs.
